SOAP notes  — which stands for subjective, objective, assessment and plan — can be very time-consuming. The Medicare manual changed the definition of medical necessity in 2009 to: “The patient must have a significant health problem in the form of a neuromusculoskeletal condition necessitating treatment, and the manipulative services rendered must have a direct therapeutic relationship to the patient’s condition.”

That means your SOAP notes need to show improvement as well as a reasonable expectation of recovery. Medicare is looking for results. Penalties have been increased for noncompliance.”
