Billing, claims and payments

A clean claim rate is the share of insurance claims accepted on first submission, without being rejected, corrected or resubmitted. It is one of the clearest measures of billing health. Platinum reports a 97% clean-claim rate, because claims are built from the signed note and scrubbed against payer rules before they go out.

Revenue cycle management is everything between booking a visit and collecting the money: eligibility checks, charge capture, coding, claim submission, payment posting, denial management and patient balances. Platinum RCM runs all of it from the same record as the chart, with a 14-day average from claim submission to payment posted.

Chiropractic manipulative treatment is billed with CPT 98940 (one to two spinal regions), 98941 (three to four regions) and 98942 (five regions), plus 98943 for extraspinal regions. Platinum suggests codes from the signed note and checks them against payer rules before the claim is submitted.

Medicare covers spinal manipulation as active treatment, billed with the AT modifier; when care is expected to be denied as maintenance, an Advance Beneficiary Notice (ABN) is signed and the GA modifier applies.
