Identity verification
We may take reasonable steps to verify your identity and authority before acting on a request. An authorized agent may submit a request on your behalf where applicable law allows.
Patients: start with your clinic
Your patient records are controlled by your treating clinic. For access or correction of your clinical record, contact your clinic — Platinum processes that data as a HIPAA Business Associate under the clinic’s agreement.

You can always submit the same request to privacyoffice@platinumsystem.com or call (888) 880-8602. To appeal a denial, reply to our response with “Appeal” in the subject line.
